It usually happens either just before the story is saved to the database, or just after it has been saved but before it has been "dugg" to your account (so it appears with a zero digg count). If you refresh the error page, depending on what happened, you may end submitting the story twice. Worse, there is no way to find out if the story made it or not, short of starting the process again!
