He had at least one nose job before Thriller, and Thriller was before he burst into flames on the Pepsi ad.
Also, his entire family denies any vitiligo and none of them have it. That's one of his early attempts at denying cosmetic makeovers. In his most recent TV interview he denies ever having any work done at all, for any reason.
Vitiligo causes your skin to appear splotchy, and the treatment is to restore the original pigment, not to bleach it out evenly.

...but, AhronZombi might have a point with the boys thing. Somehow, the cut-and-pasted MJ perving on little kids matches him a whole lot more than the no-plastic-surgery version. And to clear up any confusion he got plastic surgery, initially, because his face was burned while taping a Pepsi commercial.

If you do a simple reference line down his pupil you can see his nose wouldn't be that wide and his lower lip is exaggerated in the picture. Also, Michael Jackson's been really thin since his teens, so he wouldn't have the sagging jowls problem. Not to mention "black don't crack."
Age 45 is probably closer to how he would look if he were 60, Age 70 is closer to 75 or 80.
