There are more things that cause weight gain, then just food.
Have you ever heard of a thing as genetics? or for example "chronic fatigue syndrome"? or how about an injury that stops someone from "exercising".
Here in AZ, we have the largest ratio of obese people, and this is solely because of Pima Indians. Their genetics are obviously different, so when they eat McDonald's and such, they don't process it the same way Caucasians/African Americans do.( god knows I have to be PC, or I'll get flamed by kiddies)
Their way of life and the food they've ate for the last 1,000 years, has all a sudden changed. Their body is not use to the high fructose corn syrup and all the other "crap".

It's thin and tiny, but actually very tough. If you absolutely hate scratches, get a case. I just dropped by the Apple Store this evening, and they've got a ton of cases now.

There are several sorts of body types out there, and some people are just naturally pudgy, no matter how hard they work at it. Yeah, you're right, some people process food differently from others, and some people are fat because of medical problems, or other reasons. I know people who have had depression that led to overeating. But I really don't think that the entire one-third of Americans who are overweight have such excuses. A lot of people are just plain gluttonous, lazy, or weak-willed. Yeah, that's harsh. Tough.
I don't want anyone handing me excuses when I'm getting out of shape, that doesn't help me at all. I've certainly let myself eat too much when I'm feeling down, and you know what happens? I gain more weight, and I feel even worse. At some point you've got to stop and have some damn self respect.
Harshing on fat people is NOT the same as racism or other bigotry. "Fat acceptance" is not the kind of thing Dr. Martin Luther King Jr. would have marched in Selma for. You want to help the Pima Indians? You work for economic changes in their community, try to get rid of the McDonalds and bring back the kind of food they do process well. Give people self-respect and empowerment, that's what they need. Not some feel-good fuzzies that help in the short term, but kill in the long term.
