Replies to comments not consistently showing in notifications

I’ve noticed an issue with comment reply notifications.

Sometimes, replies to my comments don’t appear in my notifications at all. I only find them later by chance when revisiting the thread.

Other times, I receive a notification saying someone replied to my comment, but when I click through, there’s no reply visible. I’m not sure if the comment was deleted, filtered, or if the notification itself is incorrect.

This has happened multiple times, so I wanted to report it and see if others are experiencing the same issue.
