Parents are people. They aren't perfect. They make mistakes. They aren't paying attention every second of every day. Coincidentally, and horribly, she "picked" a bad time to very very brief avert her attention from the child.
Not to mention, they weren't "that close." She was almost all the way back. It is not like she was holding the baby on or past the line.
