A gratuity is a thank you (same root as the words grateful and gratitude http://www.etymonline.com/index.php?term=gratuity) I worked in restraunts to pay for college so here is my advice for good tipping: (err these are US only. I've never worked anywhere else)
1) Money isn't the only or the most important tip you can leave. Look your server in the eye. Talk to them as though they are a human being. Don't call them by their first name or snap at them. Be kind. It's the right thing to do, it's better then 2$ and you'll get better service.
2) If your service is amazing your tip should be amazing. Don't be afraid to tip more then the ticket is worth. If they were that good the tip should be too.
3) Conversely: if they were that bad don't encourage them. Everyone has bad days, but there is a difference between being overworked and stressed and being a bad server.
4)You shouldn't tip high because the place is expensive and low because it's not. I've worked in restaurant and the more expensive the restaurant the more I got paid and the less I had to do.
5) Tip before a the meal. Try it sometime. Tip half before the meal and half after. You'll get the best service you've ever had.
6) tip the cook and the busboy. It's just nice.
7) Just be nice. Tip like a nice person would tip; then you can't go wrong. - inactive, on 09/17/2008, -1/+2Great article.
