It refers to a story on the front page (e.g. page A1) continuing inside the newspaper (e.g., on page A11). On the web, it means click the link to the next page. Another dead-tree newspaper phrase is "below the fold," stories that are on the bottom half of the front page. Less important information is put after the jump or below the fold.
