Since he apparently reacts badly when they are touched, it could suggest they are very sensitive and causing him pain, and the psychological effect of growing up that obviously different to everyone you see would be crushing...

It looks like possibly the child has the condition wherein a "twin" was not completely developed. I remember a similar case in the news a while back:

If you read the article, you would notice that BOTH of his left arms are not completely developed, and that they are testing to decide which could be best suited to keep.
Parent offers an interesting question, it seems rather a tough choice to keep the third arm or not, specially if it is that well developed. Neither of his left arms are functional, and he obviously has some serious nerve issues as he cries when either of them is touched. The child was also born with a few other genetic defects (yes, defects) such as only one kidney and spine problems.
