The extra large heart is quite easy to get. All you have to do is get a group of 40 people, each with a "scroll of summoning" to visit Hellfire Citadel's rooftop and kill a 70 elite. Only one person gets the drop, though, so you may have to get several scrolls of summoning and several raids together to do it.
The scroll of summoning is quite easy to get. All you have to do is collect 10 medium hearts.
A medium heart is quite easy to get. All you have to do is collect 5 small hearts and then summon a mob easily killable by 3 people. You then collect a unique "medium heart without love" item that is on a 24 hour timer and will extinguish out if someone doesn't /kiss you within that time limit. The only catch is that players can't mutually /kiss each other, nor can people under level 70 /kiss, meaning people are trading /kisses like auctionable material.
A small heart is quite easy to get. All you have to do is run one of the Outlands instances and kill a final boss. The boss has a 25% of dropping one, so it should only take you, statistically, about 20 runs to gather a heart.
