One of them is a Medic, who is able to heal people on their team. The bald guy in the comic is a Heavy, and there has been a running joke that the Heavy and the Medic have a strange homo-erotic relationship together - due to them being such a good team in the game.
The big revelation is the final pane, which shows a Spy. In Team Fortress 2, there is a class called Spy which allows you to cloak yourself as someone of the other team and literally stab them in the back. You can also fool their medic into healing you. I believe this comic is paralleling the Spy to the various pedophiles/sex-offenders who 'fool' people on the internet into meeting them and then end up hurting them or sexually abusing them etc.
I think the key is thinking about perspective. The bald guy (Heavy) believes the person he is talking to is a hot Medic class (whom, as has already been established, shares a strange relationship with the Heavy). The Medic is, however, a Spy who is either trying to gather intelligence or merely just 'cause havok for the enemy. This is similar to in-game situations, where for example you see a Medic and you ask him to heal you. You find it odd that he doesn't start healing you until he gets behind you, but you remember that its not a rare occasion for the Medic to stay behind their Healee. All of a sudden, Bang. You are dead. The Medic got behind you to stab you, because he was infact a Spy disguised as a Medic.
It is assumed the Heavy will find himself in a similar situation as the above, if he were to meet his new online friend. I can only imagine magnitudes of trust self-destructing, damage which can only be healed by special psychiatric help. But that's another story.

One of them is a Medic, who is able to heal people on their team. The bald guy in the comic is a Heavy, and there has been a running joke that the Heavy and the Medic have a strange homo-erotic relationship together - due to them being such a good team in the game.
