I think it entirely likely that she knew exactly what she was doing when she referred to him as "Don". They've done this before. One big example that is clearly my design is to drop the "ic" from democratic as a subtle sign of disrespect. They call it a "democrat congress", etc. It's well documented. This administration (well, Rove) has a particular style in how they manipulate words for effect - calling Jon Don is a good example of how they always try to show subtle disdain for those not on their side. You know what it's like? It's like when someone has a new girlfriend who has an ex-boyfriend that is threatening in some way - rich, tall, whatever. The new boyfriend will often intentionally pretend he can't remember her ex's name and call him by the wrong name to send a signal that he's not important enough to remember his name.It fits the pattern, that's for sure.
