Your arm is never completely 90 degrees turned like in the picture. If you're like me your elbows are always resting on something whether it's the arm rests or the desk making your arm in a more natural position.
This piece of crap has been around for at least 3 YEARS. I was given one to try, tried it for 30 minutes and gave it back to the vendor. Most unconfortable and non-intuitive mouse EVER.

They do have the Left Hand version. Just do a search for Evoluent Left. <a class="user" href="http://www.svc.com">http://www.svc.com</a> has them for the cheapest (reliable) retailer I've found.
