Umbilical cords. Every baby has one and no one wants them after the baby is delivered. Sounds like a win-win to me. And they do tell you but I suspect most mothers are not really concerned about the afterbirth when they sign. It's like when you buy a house. They just keep giving you forms to sign so you sign them all.
My dad was stationed in Sicily (as part of our world garrison) and my mother had her first two babes in the 60's in a very old hospital, where she was offered her/our afterbirths!I guess women needed the additional nutrition after going through birth, and more traditional societies just continue those traditions??? A lot of animals eat their afterbirths, but that may also have to do with predator avoidance.
