well it does sound reasonably possible. If you're a cop and you're raiding a house, then you'll go in weapons at the ready, but then there happens to be a harmless old lady whom you dont want to shoot, but then she comes at you and gets into an altercation, and while you're trying to wrestle her away you accidently fire as the guns still in your hand.

@twopercent A robot. Surround the house and send in one of the bean-bag gun equipped two way communication robots that my tax dollars bought for the DPD. I guess that wouldn't be as much fun though, and the standoff might drag on for a couple of days.......Ya know what screw it. Kick the door down and kill'em all; cops shouldn't have to work overtime.
