How about the Digg V4 fiasco? It effectively killed the golden goose by alienating the vast majority of their subscribers with changes that everyone hated. To date it remains a pale shadow of it's former self and the management appears to be in shambles.

That's sad but true. I have barely visited digg during the past months - and was a loyal (although mostly passive user) with roughly 80 comments since 2006. Now this whole page (which I read daily) is in the trash. Seldom (never?) before I have seen someone destroying a successful whatever the day digg did it with v4. Digg, reboot, now! Install v3 at the coming weekend, even if that means throwing away your v4 data, just reboot this thing.
