Naked Lunch ~ for God Sakes, I was shocked and Repulsed by 1/2 the movie the first time I saw it, now it's one of my favorites.
Natural Born Killers ~ Rodney Dangerfield's dialog.
Memento ~ The End (or the beginning really)
The Killer ~ The Ending, more tragic for the characters, but shocking that they weren't reunited in their last moments.
A Clockwork Orange ~ Singing in the Rain
Easy Rider ~ The ending blows me away.
Full Metal Jacket ~ The beating in the Barracks.
Nightmare on Elm Street ~ Original was quite shocking
Pan's Labyrinth ~ The bottle to the face.
Pulp Fiction ~ The Rape Scene.
Soylent Green ~ it just IS.
Boxing Helena ~ so-so Movie, the ending was a cop out, but damn!
Tideland ~ little girl playing dress-up with corpse father doesn't qualify? Much more shocking than "You Luke/Me Father"

Honestly the most shocked/surprised I've been in a long time is when Brad Pitt gets shot in Burn After Reading.
