You aren't gonna grow up in a "hood" and play in the NHL (unless you really kick a level of ass near that of Gretzky). That's the reason most people don't associate with the sport, the access to it is very limited and most people aren't rich enough to afford hockey. Basketball on the other hand is quite easy access, at anytime, you could just pick yourself up a 25 $ ball and go to the local park and play for hours. Most schools have a basketball team. That is the simple reason to why the NHL isn't as popular as the NBA (although the quality of the action in the NHL is much higher, I hate watching basketball).

In baseball a player can't even play through a bruise of any sort. I've just heard a lot more NHL players play through injuries than a lot more other pro leauge.

The revenue part though you are bang on - the NFL has all its ducks in a nice pretty line.
