I mean, I really let myself go, but if I had ever gotten to that point, I would have killed myself.

For a man 1/3 his weight and the same age, for basic metabolic rate, he would need about 2400 calories. Every time the weight doubles the increase is by 50%, so for him it would be 5400 calories.

I know he has a long way to go but he's already holding the world record for the MOST weight loss ever etc. so kudos to him and kudos to him for inspiring others to do the same. And, yeah, he has a girlfriend ! An inspiration to Diggers then. If he can do it ..............

I don't want to speak for him, but I believe he means to feed him more nutrient rich foods than say a hearty meat lovers omelet with a side of a family serving of bacon mac n cheese.
He needs a combination of non-mobile exercise (hand peddling, etc), surgery spanning over one or multiple years as you mentioned, and low steroid dosages later on in the process near the end in order to build muscles quicker. Trenbolone acetate + Testosterone Enanthate.

Not eating = malnutrition.
Eating less does nothing if you can't exercise, and as you grow fatter, you're body has to support the systems to keep nourishment going to the extra mass. The only real answer for folks like this is medical care. Intravenous feeding, surgery to remove fat to aid mobility (at his weight would be multiple surgeries over the span of a year or more), then when he can start to move even lifting a leg would be nearly impossible with the amount of muscle degeneration.
