A lot of people smoke all their lives and never get cancer, most people who smoke never get cancer.

No, if you repeatedly abuse your skin by staying in the sun too long, you'll get skin cancer.
And if you repeatedly abuse your lungs by smoking, you'll get lung cancer.
-Nothing- is guaranteed to cause cancer. Significant amounts of some things will increase your risk to a point that it's nearly certain, though.
I'm all for people being able to do what they want, smoking and drugs included.
