Ask any gynecologist, and (s)he'll tell you that (s)he finds maybe one case every two years or so. When it does happen, it's always the same story: "Well, doc, I came in because, for the past 6 months, I've been having bleeding after intercourse. My last pap smear was maybe about 10 years ago. I remember they called me and told me it was abnormal and I needed to come back in, but I just never got around to it."
Once the HPV mediated changes have advanced to invasive cancer, then you've got a real problem, but that takes several years, and up until then, cervical dysplasia (precancer) can be fairly reliably detected by the Pap smear, and in most cases the process can be arrested before it's too late.
The only commercially available vaccine in the US is Gardasil, which only protects against infection by four of the dozens of human papillomavirus types that exist. Gardasil protects against types 6 and 11, which cause genital warts, and types 16 and 18, which together account for about 70% of cases of cervical cancer. The chance of transmission via a toilet seat is essentially next to nothing (although I still hate public toilets). It is true however that many STDs can be acquired via routes other than sex.

Are you implying that orgies being in style and marriage being out of style in Rome single handedly led to the collapse of the Roman Empire and that it was because of STDs? I'm not sure that's a very plausible or supportable claim but I'm no history expert so correct me if you like.
Your analogy with "Children of Men" is interesting. Although I think that this theoretical situation of a manufactured epidemic could just as easily be a disease that is spread through means other than sex, and would likely be much more effective in other forms as well. Just because a disease has an effect on human reproduction doesn't mean it has to be transmitted through sex.
As a twist on your doomsday idea, imagine that for whatever reason the world in "Children of Men" became reality and humans couldn't reproduce. Now what would happen if sex in that 'doomed' world was only allowed in marriage? I think it's a reasonable assumption that there would be far less sex going on than in the same 'doomed' world with massive sexual promiscuity. So which would be better? We would certainly want to get back to being able to reproduce, and more people having more sex more often seems to be the option that would lead to a greater chance of finding some combination of man and woman that were able to reproduce (i.e. a couple with natural immunity). Just food for thought.
But overall I think the actual threat to human kind from any current STDs, even HIV, is pretty low compared to the wealth of diseases that are much more contagious. People have been having sex out of marriage for all of recorded history, and young people have been having sex all that time as well. Besides the fact that it's normal and there's nothing wrong with it at all, there's also the fact that you just can't make young people not want to have sex. Since it's so pointless to try and force abstinence on people, we can at least provide education and access to technologies that can prevent the transmission of STD's.
Lastly I'm not so sure about your claiming that 'most cultures around the world treat sexual promiscuity outside of marriage as a big social taboo.' In most of the developed world, I'd have a hard time believing that anyone other than very religious people see it as much of a taboo. - schnikies79, on 10/10/2007, -0/+6There is no sure-fire to test males for HPV. If a male develops warts, those can be tested, but otherwise you really can't. It's always a good idea to be tested for other STD's though, and it's usually free. Many STD's are 100% treatable.
