It's like a sandblaster for powdered medication that shoots it through the pores in your skin with a high presure blast of nitrogen. Armed forces used to use them for mass vaccinations. They hurt like a biatch, and were notorious for causing infections in the sites.

The doc pushes only on the inner filament so when in the skin tissue it binds and pushes the needle along with it. Once the tip breaks into the cavity the filament can move forward again so the doc doesn't feel any more resistance and the outer needle stays put.
