The maggots used in the NHS are grown in Switzerland. You don't actually have them in contact with the skin. They are held in a packet with an extremely thin porous material which they can eat through, in the NHS anyways ;)

They were interviewing a patient with a nasty diabetes related ulcer that wouldn't heel and he was shocked when it was suggested, but as nothing else had been working, he gave it a shot. His one stipulation was that nobody was told about it. They don't burrow around eating your flesh; they burrow straight in, and then remain immobile as they suck blood from your deep layer of skin. After a few days they just crawl out and fly away. What makes them valuable to medicine is that they secrete a potent antibiotic and a pain killer into their host's tissue while they are present.
