Studies like this are done in the following manner:
1. They choose a high risk population such as prostitutes.
2. They advertise they are doing a vaccine study and want a few thousand participants.
3. They educate, inform, and obtain consent from all the participants that they may be getting a real vaccine OR a placebo (dud fake injection). Participants are not informed which one they received.
4. They educated ALL participants equally that the vaccine may not work, and furthermore that it's quite possible that they may have gotten a placebo instead of the real vaccine. They strongly advice to use condoms and ideally abstain.
5. After some years they see whether there is a difference in the amount of HIV cases in the vaccinated group versus the ones who go the placebo. Since the thousands of participants are sexually promiscuous people, there is a high likelyhood that many of them may have caught HIV and ignored the education.

They simply don't like the extra expense of providing health care for their employees and would rather shift that burden on the US tax payer. Furthermore, anyone who creates a cure for AIDS/Cancer is going to be rolling in money. If there was no money in the prospect of a cure, then no private company would have spent money trying to find a cure. The reason why we fail so badly at finding a cure for AIDS is because of how quickly it mutates and the number of ways it can infect cells while bypassing the body's immune system. As for cancer... you speak as if its one type of disease. The variety in cancers of the same organ is mindblowing, and cancer is an INEVITABLE outcome of animal cells. Simply put, if you live long enough... something will break down and you will get cancer. Any "cures" for cancers are going to be specific towards a certain mutation in a certain type of a cancer.

Just like in the US, with all the education about HIV, the infection rate every year isn't 0. So they give the vaccine/placebo to everyone, and see which group ends up with more cases of the infection.

On a more serious note this could have been done in retrospect with known infection rates for this particular population however the sample size would have had to be much much larger to compensate for the obvious bias of having different conditions, and then again your study would risk not being taken seriously.
