Try having your doctor send the next one to walgreens/walmart instead... if it happens then you might want to get a new doctor

I dont know how; since my Dr. NEVER faxes to them or and I never authorized them; but EVERY time I get a prescription; I have it faxe3d to my local CVS; I pick up my script and think its all good... usually like clockwork 3 days later a box from ExpressScipts appears full of the medicine that i already picked up; and with a VERY high invoice attached.
CVS doesn't know how they get the script; my DR doesn't know how they get the script...

Its interesting the amount of information you have access to. One could just write down this info over the course of a day as they fulfill prescriptions. Every time a script is filled it ends up in the PBM's system for claims adjudication. The PBM then adjudicates the script claim and facilitates payment to the retail pharmacy.
The problem with your particular scenario is that the PBM doesn't just send out medications based on the claim information. They actually have to have a script in hand from either the doctor or a script explicitly transferred from the retail pharmacy in order to enact an order.
The only other way this could happen is if your insurer has set up Exclusive Home Delivery with the PBM. In that case you wouldn't be able to get more than one refill locally and the pharmacy would let you know that when you filled.
