In this documentary I saw, they followed a few cases. One that they blocked was a woman who wanted both her legs cut off. They blocked her because she wouldn't agree to have just one cut off, it had to be both. It seemed that she had a kind of "disablement envy" and thought she would be happier if she was in a wheelchair getting everyone's sympathy. While I think a fair percentage of the population would consider a sex change morally acceptable, I think a far smaller portion of the populous would agree with cutting off a patient's leg because they were unsatisfied with it.
