The downside to being an abrasive, is after continued use it will start removing your enamel, and it will start scratching your gums (especially if you use a Medium / Hard bristle brush).

Acids dissolve the calcium out of the enamel; any acid based "whitening" will do this, which is why no self respecting dentist will recommend it. As an added bonus, you are pretty likely to induce (possible severe) tooth sensitivity.

Water didn't help it because he didn't neutralize the pH to a comfortable level. If you take Lye (pH 13.6) and something like Vinegar (3pH) you can better neutralize the pH. Actually, Coffee beer and orange juice all are worse than Malic acid. I think the agent to be worrying about in this is the baking soda which has a 9pH. Even though it would neutralize the solution, you use more baking soda than Malic acid.
