It's just that the magnet stimulates whatever nerve endings it touches to fire, so it feels like your scalp is getting pinched every time it goes off. It's fine at first, but eventually gets really, really annoying.

Appetite, unsurprisingly, has received a lot of attention in neuroscience, and (given that our understanding is still limited) most signs point to the hypothalamus as being of primary importance. TMS can't penetrate the outer cortex, and the hypothalamus is pretty well covered.
