I have a death to add to the list though:
Quake 1. This happened on Q3DM3 all the time (Abandoned Base). You get the pent (invincible) and the lightning gun, hop in the water, and fire up, fragging anyone else in the water. Every time you saw that evil glow coming through the tunnel from the pent into the water, it was a frantic race to get out of the water. Good times. - addiggt, on 07/06/2009, -8/+30Post Death Teabag. In fact I can think of more deaths that top quite a few on that list (except for the immaculate boob fatality from Killer Instinct).

- Mitijea, on 10/12/2007, -2/+8No, the worst way to die in a video game is when, say in one of the original tomb raiders (but there are many examples), when the camera got messed up and you thought you were jumping toward an easy ledge and instead landed with a snapped neck. Death due to poor camera or controls is by far the worst.

They should have gone for more humiliating ones like dieing on the first level on mostly any game (after you've been playing that game for a long time).

The bonus would be dumping your regenerative health power-up on the victim to keep him alive for as long as the time limit or until he dropped.
