(might have been one of the R-Type games, but I'm not sure) There was a teenager playing a scary video game. He was really getting into the game when he yells out as he feels a hand on his shoulder. He calms down as he realizes it's his mother. Then the hand starts brushing back and forth limply on his shoulder. He says 'Mom?' as the camera pulls back and we see that the dead arm is being held by a monster hand. Kid screams, fade-to-black.

He flies out the window, and as he flies over the terrain he ages from baby to child to adult to old man ... then he plummets into a grave. The tag-line "Life is short ... play hard" comes on screen, then you see the Xbox logo. Sorry I don't have a link.

It features a teddy bear that looks an awful lot like the "Snuggles" bear from Fabric Softener fame. It was only on for a couple of weeks until Lever Bros (parent company of Snuggles fabric softener) forced 3D0 to pull the commercial!
