Most cities in Italy (at least the ones I've been to) have fairly ancient plumbing, which can sometimes smell a bit. It's not quite a ***** smell like someone just took a dump in front of you, but a smell that you'll recognize as a sanitation processes.
The real annoyances in Italy are transit strikes, organized crime (in the south) and Romany thieves in the tourist areas. Compared to that, the occasional whiff of toilet tank is nothing. 2. People who sell things to tourist.
