Uni-Form
dnevnikeklektika.com — Uni-Form is an attempt to standardize form markup (xhtml) and css, "modularize" it, so even people with only basic knowledge of these technologies can get nice looking, well structured, highly customizable, accessible and usable forms.

In your example this is not VB6 but instead, an assembler code snipplet.
I have my own "standard" for making forms, I use it every time and it, for me, is drop in place and off I go. Most others who know and have worked with forms probably have their own way of doing it as well with likely none being that much better than any other.
This is nothing more than a snip of code that people can copy and paste, that's all.

One would probably need a web site too to go along with a form, should have added it because a form is worthless without a website to put it in. ;-)
