Starvation is a side effect of not having access to food, lack of access can be for many reason. obvious is there is no food (but that is the least likely), other reasons are: corrupt organizations blocking it or rerouting the food, nobody willing to ship the food there at any price (like through a war zone or a plague area), too far from agriculture centers for consistent delivery, inability to store food long term, or in ability to move food to the people that need it before it spoils.
