Clone stamp was used in order to help chop off a little extra skin and to smooth out the fold in her skin.
That's a rough overview. Seriously, the best way to learn to do this is just to play with photoshop for a while. In my younger and more vulnerable years, I used to make naked pictures of girls in my school just to learn how to use Photoshop...a few of them saw the pictures, and a few of those girls were fine with it, since I was fairly generous with how I portrayed their bodies. The rest of them filed restraining orders.

Actually, what they usually do is find a normally athletic person, who has suffered some kind of injury forcing them to no longer be able to keep their training regime. Once they're healed (and have gained lots of weight) they take a "before" photo, wait a few months for them to lose all their weight, now they can train again, and take an "after" photo.
