The dictatorship's authorities will keep trafficking with prisoners' organs (even when they have no death sentence) until something or someone takes the dictatorship to an end.

Once they become a commodity worth money (LOTS of money), then THAT is an incentive for governments and corporations to supply the market with what it demands. Where do you draw the line? At murder? At rape? At theft? At driving-without-license? At smoking pot? At littering?
Remember, we are talking about a "product" that prolongs peoples lives. If those people are tax-payers and the state offers them a chance of life at the expense of a few felons, then pretty soon those tax-payers would happily approve the death-penalty for just about any crime on the books.
If the population demands transplant organs, and the law allows them to have cheap access to them via state-sponsored "organ harvesting" prisons, then THAT is a recipe for totalitarian hell.

He also warned of the consequences should the demand for such organ "donations" become commonplace. A populace desperate for cheap transplant organs and longevity would approve the death-penalty for previously "trivial" crimes, like tax-evasion and jay-walking. Something to think about...
