I understand that he can spin the image by swiping the screen, so he could have set the original orientation to be facing the camera, however on a flat table the accelerometer wouldn't be able to distinguish the rotating motion that accurately, would it?

Anyways, iPod Touches and iPhones do not sense rotation on only the Y axis(oddly, this is only limited to the Y axis). To change from portrait to landscape, you not only have to rotate the device, but also tilt it slightly downwards. This limitation is obvious if you have a sensor reader app like the ones available on the old jailbroken 1.1.4. To sum it up, the iPod/iPhone accelerometer can only sense movement on the Y axis if there is movement on the X axis or the Z axis, assuming we're using the standard coordinate plane thing. - rbd89, on 08/23/2008, -0/+13@AlPacinoTribute

MobileSafari simply ignores what the accelerometer is telling it about that axis. The developers could have accounted for that axis in some way if they wanted.
Straight from the manufacturers: "The LIS302DL is an ultra compact low-power three axes linear accelerometer."
Note that it handles rotation around all three axis, not just two. And that it's a linear accelerometer, so it measures relative acceleration around each of the three axis.
